Sub-Acute Care

Most patients cannot wait to leave the hospital and return home. For many patients, however, going home is not an option because they are not yet physically able to return to their daily activities.

1.14.subacuteIn Pine Valley’s Sub-Acute care unit, patients receive high level care, coupled with complex physical or medical rehabilitation from a physician-directed interdisciplinary team. Pine Valley’s team of doctors, nurses, therapists and other health care professionals work with the patients and their families in developing a goal-oriented plan of care for each resident. The goal of the plan is to help the patient achieve their highest level of independence as quickly as possible and to ultimately return home.

Since Sub-Acute care is generally more intensive than traditional nursing facility care, Pine Valley is fortunate to have not only the skilled professional care of the nurses and the therapists, but also the sophisticated equipment and technology to help patients recover faster.
